Valid government-issued photo ID
High school diploma / GED transcript
3 Letters of recommendation (for scholarships)
Immunization records (Hep B, MMR, Varicella, TDap)
Current CPR card
Level II background check
$30 entrance examination fee
Proof of US citizenship/residency
Professional Goal Statement (1 page)
Completed physical / health documentation
Current PPD / TB test or chest x-ray
10-Panel Drug Screening
Medical insurance proof
